Indications/Uses
Anaesth techniques, including local infiltration, minor & major nerve blocks & epidural block.

Dosage/Direction for Use
Individualised dosage. Adult 400 mg over 24 hr. Childn Should be calculated on a wt basis up to 2 mg/kg.

Overdosage
View Marcain Adrenaline overdosage for action to be taken in the event of an overdose.
Contraindications
Hypersensitivity to amide type local anaesth & Na metabisulphite. IV regional anaesth (Bier's block).

Special Precautions
Cardiac arrest. Acute toxicity effects on the CNS & CVS. Ventricular arrhythmia & fibrillation. Elderly & patients in poor general condition. Patients w/ partial or complete heart block; w/ advanced liver disease or severe renal dysfunction; in late stages of pregnancy; treated w/ anti-arrhythmics class III. Epidural anaesth may lead to hypotension & bradycardia. Patients w/ severe or untreated HTN, poorly controlled hyperthyroidism, ischemic heart disease, heart block, cerebrovascular insufficiency, advanced diabetes & any other condition that might be aggravated by adrenaline. Contains Na metabisulphite that may cause allergic reactions.

Adverse Reactions
Hypotension, nausea, paraesthesia, dizziness, bradycardia, HTN, vomiting, urinary retention.

Drug Interactions
Additive systemic toxic effects w/ lidocaine & mexiletin. Anti-arrhythmics class III eg, amiodarone. Severe, prolonged HTN w/ TCAs; concurrent use of adrenaline-containing soln & oxytocic drugs of the ergot type. Reduced or reversed pressor effect w/ neuroleptics eg, phenothiazines. Risk of serious cardiac arrhythmias w/ inhalation agents eg, halothane & enflurane. Enhanced pressor effects w/ nonselective β-blockers eg, propranolol.
